Please note that the ER38-18D is derived from the ER38-18. The smaller flange is missing to facilitate usage in larger and shorter ear canals where the smallest flange would cause discomfort. Rather then specifically manufacturing the 2-flange unit only the ER38-18 is manufactured and appropriate flange is removed in-house under sterile conditions.
